Policy and Procedures
In accordance with Indigenous Service Canada (ISC} policies and procedures:
1. LSLB Membership Clerk does not make decisions regarding Indian Status
2. Issue Certificate of Indian Status {CIS)
3. Assist with documentations for Secured Certificate of Indian Status (SCIS)
4. Report life events such as Marriage, Divorce, Births, Deaths, Adoptions etc.
5. Maintain current Band list and accurate membership records
6. Provide all current information to band membership regarding changes to policies and/or procedures
7. Assists with documentation for upcoming elections such as voters list

Forms
Secure Status Application Form - 16 and older
For applicants 16 or older, you must submit:
Application for Registration on the Indian Register and for the Secure Certificate of Indian Status (SCIS) (for adults 16 years of age or older)
Secure Status Application Form - 15 and under
For children (15 or younger) or dependent adults, you must submit:
Application for Registration on the Indian Register and for the Secure Certificate of Indian Status (SCIS) (for children 15 years of age or younger or dependent adults)
Secure Status Application Form - asserting parentage
If you are asserting unknown or unstated parentage, you can submit:
Statutory Declaration for Biological Parents Affirming Parentage
Secure Status Application Form - asserting parentage
If you are asserting unknown or unstated parentage, you can submit:
Statutory Declaration – Unknown or Unstated Parentage
Secure Status Application Form - first, renew, replace or update
If you are applying for your first secure status card or to renew, replace or update your secure status card, you must submit:
Secure Certificate of Indian Status (SCIS): Application for already registered persons
Secure Status Application Form - guarantor
If you need a guarantor, you must submit:
Guarantor Declaration: Registration and Secure Certificate of Indian Status (SCIS)
Secure Status Application Form - guarantor
If you can't find a guarantor, you must submit:
Statutory Declaration in Lieu of Guarantor: Registration and Secure Certificate of Indian Status (SCIS)
